Dear mummies, i am speaking from real experience and would like to caution all out there <B>not</b> to visit the mentioned pd and/or discontinue all follow up.

My 5 week old baby had a high TSH (thyroid stinulating hormone) reading in her blood at birth and a repeat test before we went home showed an improved but still out of range reading. The assigned pd at TMC was Dr KT Low then who requested us to follow up with him at his clinic at 6 days old.

At the follow up visit, another blood test was done and a day later doc called and said reading is improved and everything is ok.

During the follow up, we mentioned that we will be following up future visits n vaccinations w another pd due to proximity to our house. (And of cos paragon clinics has generally higher charges not to mention the parking.)

A couple of weeks later we received the test results which still showed TSH out of norm range (this time the norm range is different from at 3days old due to age of baby). We did not read much into the out of range result as dr low said everything is NORMAL.

Last week we went to another pd at tampines for her hep B jab, dr looked at her tsh and requested a blood test to be done again. We were perturbed but of cos went ahead (despite the heart ache seeing her blood drawn again).

Next day dr called and told us results are still not normal and we must go to nuh the following day with endocrinology pediatric specialist for the first mornin appmt.

Fyi all mummies, a high tsh reading could mean coginental hypothyrodism (CH) and early treatment is crucial as any delay would affect growth and development ie. Iq points could drop significantly.

Anyway we did the follow up, specialist prof loke scheduled an immediate thyroid scan. Luckily her thyroid gland is normal but her high tsh reading meant she has CH and required mediciation and blood test regularly till at least 3 yrs of age.

As worried as we are, we have to move on with this and hope her follow up test will show normal readings with this thryoid hormone replacement drug.

Today i decided to call dr kt low and hear for myself his intepretation of the 6 day blood test again. (He spoke to my husband then) Over the phone, he said in his oh-so-casual tone that

Doc: oh everything is normal, her reading has dropped so nothing to worry
Me: but her result still show not in norm range so in ur opinion no need another blood test?
Doc: her birth results r very high and 3 days later dropped alot, so this result dropped even further so <b>SHOULD BE OK LA</b>
Me: i ask the same question again
Doc: no need, cos if u apply insurance, sometimes they will request for another test to be done looking at the out of range results. So by then u can discuss with ur pd again lor.
Me: ok thanks

I was FUMING. I kind of regretted not scolding him on the phone but heck, actions speak louder and im requesting for ALL mummies to take note of the above and NOT to patronise this doc.

My own intepretation:

1. His diagnosis is based on assumptions: a trend of improved readings does not signifiy tt things will be OK. The 6 day abnormal test somehow did not push him to get us to follow up promptly

2. Lack of doc ethics: my gut feel is that, knowing tt we will not follow up w him, he has left the job for others to be done. Despite the importance that this is a TIME URGENT medical issue.

Seriously i cant imagine what will happen if i choose to continue with dr low as the next visit would be at 3 months of age and i cannot fathom what will happen..

Sorry for the long read but PLS SPREAD THE WORD around..our child's life cannot be left to the pd's hands only.

Hi aisui

I feel for u. My daughter has CH too. Such conditions warrant immediate medical attention since it could affect her development. My daughter showed an abnormally high tsh at of 400+ at birth, and my pd did a blood test again for her at 3 days old. Though it went down significantly to 100+, the pd was still concerned. She was immediately put on thyroxin on her 4th day to ensure that her development would not be affected. My pd ran another blood test on her 4th day too and tsh jumped back to 400+. A gd pd will ensure that all abnormalities are identified and recommend treatment ASAP.

Now tt you're seeing dr Loke, you can rest assured he's a gd doc. I went to him for second opinion and he's very patient and knowledgeable. I'm currently with endocrine doc at kkh for my girl's CH. my girl is now 7 months old and like any other baby.

This is scary. Anything that goes wrong will affect the child whole life.

I would also like to add that dr low does not give ANY eye contact while talking to us and its really irritating.

And rude.

My daughter was assigned to dr low. I don't want to repeat my series of incidents but I must agree he is too casual with his diagnosis. Maybe his rates are cheaper and a lot of people are going to see him.
A couple of major incidents had me stopped going to him which I reegretted I should have moved on. I'm now seeing another PD.
Yes it is slightly more expensive but it has guaranteed full attention and specialist to seeing my daughter's well being in her early life.
It's definitely worth it for that little bit more money to see the little one less in pain and happy. I saw my daughter improved tremendously when the new PD just 6 months:)

Dear Mum,
I feel for you and am sorry that your daughter has been diagnosed with CH. I can imagine that having to sort through medicines for the next three years will put further stress into an already tough situation, but it is great news to see that it will only be for a few years and that you indeed have a healthy baby that is on the mend!
I am concerned though that you would speak your frustration as you have given your supposed experience with Dr. Low and ask others not to see him or to stop seeing him as he truly is one of the finest pediatrics in SG. I also have first hand experience with Dr. Low and I have to say that my experience is completely opposite of yours. Especially on your point regarding what you interpret to be casual non-chalant attitude that warrants ethics review, I see as great bedside manners that helps to calm anxious and nervous new and existing parents.
I too have had my scare with my last child who was born with something similar to wet lung and Dr. Low immediately had him rushed to PICU at Mt E where he stayed for a week, essentially to dry up his lung. During this time, Dr. Low cared greatly for my son and as a neonate specialist who has seen thousands of babies and children he monitored his stats beautifully and only performed tests when tests were warranted, something that as a three-time parent I can appreciate as you too state can be traumatic to the child (blood being drawn is so sad to watch!) He was also quick and ready to offer the means of bringing other doctors on board for additional review of X-rays and tests if we wanted. And through it all, he was great at reminding us that all was ok as he meant that given his professional medical expertise.
Which brings me to your point-- if you felt unsure at the time of release about your baby's test reading, you should have demanded more tests and/or scans done before the release. While I was in the hospital, Dr. Low provided detail information on the situation re my son's lung, which allowed me to read up on it and do my best to learn about (although you take what is said online with a grain of salt). Given that many children in previous years DIE from what my son had, you can imagine that I wanted to know as much as possible about all treatments and I ensured that I got it. So, the onus is up to you and not the doctor to ultimately speak on behalf of your child. From reading what you state about what the doctor noted, I feel that the doctor did zero wrong based off his medical judgement and knowledge and experience (yes, this man has seen thousands of patients, with many of those having similar experiences to the one your daughter) and this is more a case of personal styles not fitting.
I am and will continue to be very pleased with the level of care and expertise that Dr. Low provides to my children and I wish you the best of luck with your daughter's continued road to health. I should note that our family comes from a line of doctors and nurses, so we do have experience on dr skills and also dr bedside mannerisms. I also urge parents to speak up more for their children and at a greater frequency and to demand more tests if you truly feel that you should. You are responsible for your child at the end of the day. Good luck and here's to happy parenting of all children!
