Anyone had change name for your baby? What is the procedure?

For bbs 1 year and below, just go to ICA to do the change. They will take back the original bc and issue you with an extract (provided you have not requested for extract before).

Does the extract looks like birth cert, comes with laminate?

Yes, but smaller in size. It will state Extract of BC
